Discharge planning, interdisciplinary collaboration, and the role of the nurse

Abstract


This paper exams the role of the nurse within the context of the interdiscplinary healthcare team in discharge planning for a patient hospitalized with spinal tuberculosis and experiencing multiple comorbidities including HIV positive status, mental illness, and long term injection drug use. Of particular focus is the delivery of patient-centred care based on the unique context of this individual patient.
